Immediate Changes To Campus Check-In Process

Due to changing conditions related to the COVID-19 pandemic, Los Angeles Community College District (LACCD) and Los Angeles Southwest College (LASC) are updating the way you will be able to come to campus. Effective Monday, May 3, all employees, students, and visitors who want to get onto LASC's campus will need to complete an assessment form detailing any COVID-19 symptoms they may have as well as possible points of contact. The form will have to be completed each day before you arrive on campus.
The form can be completed by using LACCD's SAFE app, which can be downloaded in the App Store for iOS devices and the Google Play Store for Android devices. To submit your assessment via the app, click on the COVID-19 banner on the app's homepage, next click on "COVID-19 self-assessment," then click whether you are a student/employee or visitor, and fill out the form.
You can also complete the form on your desktop or laptop by visiting www.laccd.edu/covidcheck.
This new check-in process will support LACCD and LASC as well as public health officials with their contact tracing efforts. Contact tracing helps identify and communicate with any person who might have come in close contact with someone who tested positive for COVID-19. This new check-in process was approved by the Los Angeles County Public Health Department. The state Legislature has mandated that it stay in effect until 2023.
Sheriff's cadets will continue to be staffed at LASC's Western Avenue entrance to ensure the COVID-19 assessment form policy is being shared and followed.
Access to LASC's campus continues to be limited for employees and students. If you need to visit campus, please also fill out the college's Return to Campus form to receive permission to come onto campus. The form can be found at this link: https://form.jotform.com/202026135978153.
